e have a pretty decent G4 here at work that identifies itself as
"Machine ID 406, Powe Mac G4 AGP Graphics." But we're experiencing
some flaky, random crashes, especially when running OS X, and it seems
like it would be a good idea to run Apple Hardware Test.

Like many machines in work environments, this one has been in several
different departments and has been in the custody of several different
people, some of whom have left the company. The original accessories
box and "Apple Hardware Test" CD for this machine can't be located.
(And, needless to say, the Apple Hardware Test CD's we CAN locate are
for subtly different models and will not run on this one.)

Anyone know how to obtain a replacement?

I couldn't find one on Apple's website; it does not appear to be
available for download.

Try calling Apple's tech support to ask for a replacement
hardware test CD. That would be the logical thing to do.
