Flash Simm software?

Hello
I have en old HP Laserjet 4/4M that years back had a HP Flash Simm put in.
This was used for printerforms (invoices etc) and it worked fine.

I haven't used for som years but want to use it again and store new forms in
the Flash Simm. Unfortunately I can't find the floppy disc with the old
software I used to transfer the forms on to the Flash Simm. I tried with a
new version called "Forms and Font Manager", but it doesn't work. The forms
are not stored in the Flash Simm.

Should any one out there have the software for the Flash Simm and can send
it to me I will be very greatfull.
