HARD DISK PROBLEM...

my computer restarts automatically and i tested the devices one by one
(Ram,CPU,Mainboard,Power, Hard) and finally i realized that my Hard
disk causes the problem. then i found an application named ACTIVE
SMART. it lets you know about a potential disk health problem before
you lose valuable data. sometimes it alterts that something's changed.
and the log file contains:

what's the meaning of the alerts shown above? is my hard disk damaged
and needs to get fixed?
