OT: How do you guys keep track of the registry changes and patches that you apply manually

I am asking if any of you guys have some kind of software tools that
keep track of the changes that you make.

The idea is that this way you can be sure that all your systems are
patched and modified consistently. And if you upgrade or reload your
OS, you can be sure to apply all the changes necessary.

I use a text editor ;-) Every system has a comprehensive running log file, a
copy of which is stored on two other systems. Every change is logged and the
remote copies updated.

Also - and at least as important - every system has its boot/application
partition imaged regularly, with a copy of the image file stored on a second
system. Crap Happens - and I long ago lost count of how many times I've
restored systems...
