Had a similar problem a long time ago (actually, it was a year ago, but
still...) the problem with mine was a blown resistor on the card. This
usually only happens when you have a power surge. Even if you do have a
surge protector, stuff like this can happen if the phone line is not
surge-protected. the best solution is to look for a surge protector
that will also protect the phone line, thus also protecting your modem.

How to find out if anything is blown on the card? look at where the
phone line in connector (specifically the pins on said connector)
connect to the PCB. usually the first thing one of them runs to will be
blown. If this is the case then head on over to http://newegg.com and
buy a new modem and surge protector there (one of the ones with phone
line protection)
