Is a web form response too much to expect?

I filled out
http://www.pcchips.com.tw/PCCWeb/Sup...uID=49&LanID=0
due to problems with a PCChips T18 4 times in the past 5 days. I've yet
to receive even an acknowledgement that this was done. Has anyone had
any success with PCChips support? What is required in order to get
support from them?
